Please notify us if you have changed your name or address. You will need to provide us with the necessary documents before any changes can made to your record (A utility bill with name and address showing and one form of photographic identification). If you reside outside of the Practice boundary you may be asked to re-register with another Practice. Our receptionists will advise you.
Please let us know as soon as possible if your address changes.
